That is a dramatic swing in the wrong direction. And it challenges the popular belief that any low-carb diet is automatically a healthy one.
The Low-Carb Diet That Actually Works
The same study found the opposite result when people swapped carbs for plant-based foods. Those who cut refined carbs and replaced them with whole grains, legumes, nuts, and plant proteins cut their diabetes risk by 16%. A separate 20-year study of over 82,000 women found that women eating low-carb diets rich in vegetable fat and protein had a 20% lower diabetes risk than women on high-carb, low-fat diets. The pattern is consistent across decades of research.
Plant-based low-carb diets also showed a 24% reduction in early death among people already living with Type 2 diabetes. These are not minor lifestyle tweaks producing minor results. The gap between the best and worst low-carb approaches in this research is enormous, and most people choosing a low-carb diet have no idea that gap exists.
Not All Carbs Are the Same Either
The study also sorted carbohydrates into healthy and unhealthy groups, and the findings are worth knowing before your next grocery run. White potatoes ranked as an unhealthy carb because they spike blood sugar fast after eating. Sweet potatoes, by contrast, contain fiber and carotenoids that slow that spike. White bread, white rice, and sugary drinks fell into the same problematic category as white potatoes, while whole grains and legumes landed firmly in the beneficial column.
This distinction matters because many people assume a low-carb diet solves the carb quality problem automatically. It does not. Someone eating a bunless burger with bacon and skipping the beans is still making a high-risk dietary choice, according to this data. The researchers linked red meat specifically to higher diabetes risk through saturated fat, heme iron, and a gut byproduct called trimethylamine N-oxide. These are mechanisms that do not disappear just because someone skipped the bun.
What the Research Cannot Tell You Yet
This study has real limits worth knowing. The participants were U.S. medical professionals, a group with higher health awareness and better access to care than average Americans. Their diets may not reflect what most people actually eat. The researchers also used food frequency questionnaires, which rely on memory and can carry errors. And the study did not include enough strict ketogenic dieters to draw conclusions about very low-carb plans under 37% of calories from carbs. Those gaps leave important questions open.
Still, the core finding is hard to dismiss. Three independent groups of people, tracked across 30 years, produced the same result. Low-carb eating built around plants protects against diabetes. Low-carb eating built around red meat and animal fat does the opposite. The science here is not murky. The public messaging around it often is, but the data is not.
